Technical specifications:

PAXForms™ Online requires only an internet connection and web browser. PAXForms™ uses required fields to ensure complete records, and can be customized for local conditions and regulations.
A mobile version of PAXForms™ is available for use in field work. PAXForms™ operates on most PALM OS™ handheld unit (PDA) for data entry by field personnel.
Currently PAXForms™ is being utilized in several states for various tobacco control and youth access programs. PAXForms™ enables clients to preload the addresses of sites to be visited, even customized by geographical boundaries. Currently the PAXForms™ software records four types of visits to retail sites selling tobacco products:
  • SYNAR checks
  • Reward & Reminder™ checks
  • Education visits
  • Feedback visits

Additional areas for adaptation include, but are not limited to:
  • Alcohol compliance
  • Case worker data entry
  • School violence prevention programs
  • Automated geo-positioning (GPS) stamping of each record
